Healthy Village is a health, wellness and social needs navigation tool created by Claremont Healthy Village Initiative (CHVI).
The app allows residents in the Claremont Neighborhood to:
1. Access resources available in the community
2. Learn about various events and register
3. Complete a needs assessment to see recommendation
4. Manage their health and social needs.
